Ordering online from Unique Wholesale Jewelry is very easy and convenient. To request your new account, please fill out the form below with your business information. You will receive confirmation via email when your account has been approved. Unique Wholesale Jewelry accepts all major credit card payments for merchandise purchased.

Only retailers and wholesalers may set up a Unique Wholesale Jewelry account. All new accounts must be approved prior to any order being shipped. A copy of your business sales tax permit must be emailed to [email protected]  or faxed to 1-866-429-4126 with subject line your business name. A federal Tax Identification Number is required to set up an account. If your business is located, in Louisiana, you must also submit a Louisiana Resale Certificate Form. If you are applying for credit, a credit application must be completely filled out and mailed. along with a copy of your sales tax permit to the following address below for processing. Please allow 4 to 6 weeks to process and approve your business credit applications.
